To access most of the free services you do not need a referral just telephone for an appointment.

This service provides a multi-disciplinary assessment, information, advice and assistance to people requiring aged care services (and their carers) in the Auburn Area.
The service is free, and referrals are accepted from anyone. Older people considering moving to hostel or nursing home accommodation can contact the team for advice and assessment.

The Nursing Home and Hostel Liaison Service (NHHILS) maintains a waiting list for residents in the St George Area assessed as needing residential or hostel care.
The NHHILS works with the St George Aged Care Assessment Team (ACAT).
Aged Care Assessment Team (ACAT) - Concord
Aged Care Assessment Teams (ACAT) assist older people and their carers to find appropriate aged care services when they can no longer care for themselves at home. ACAT provide a free comprehensive assessment of care needs at home or your local hospital.
An ACAT assessment and approval is required before people can access residential care and government aged care programs such as Community Aged Care Packages (CACP) and Extended Aged Care at Home (EACH) Packages.
