Dementia care plan hopes to improve hospital visits

Hospital staff in the UK looking after people with dementia will have access to a new leaflet which aims to improve care for people with dementia when they are admitted to hospital. The leaflet provides a 'snapshot' of the person with dementia and describes an individual's likes, dislikes, habits and hobbies.
