Home medicines review

A Home Medicines Review (HMR) is a service that assists people living at home to safely manage their medications. A pharmacist visits the home and along with a doctor reviews all medications a person is taking.

Home modification

Home modifications are adaptions made to the home to allow people with physical limitations and their carers to help them live safely and independantly.

Home and Community Care Program

The Home and Community Care Program (HACC) is a government program that helps older people stay at home longer. This program provides services such as meals, transport, basic nursing care and home modification. 

High-level Care

People who require high- level care or who have been assessed by the aged care assessment team (ACAT) as needing high level care need extensive assistance with daily living activities and would generally require care in an aged care home.
