Talking about dying

I recently put my mother in a nursing home after a stroke and was faced on admission with questions about advanced care plans and which funeral director I preferred. Whilst I understood the need for such information found it very difficult to talk to mum about it at an already emotional time. Would suggest anyone have discussions about death and dying before disaster happens. I was wondering about the validity of these advanced care plans does anyone have any info? Thanks
